Add Report for Log in per position per user

I would like to run the reports ongoing to see when panel members last logged in for each position. The report would be more efficient than to check each individual user per position

Remove referrals and referral flags

In the Referral edit window, have a "Remove/ delete" option and when selected, also asking for a "Reason" that is mandatory free text field. This should be recorded in the history for record keeping and reporting purposes.
