Application history - email - include name of the applicant

re-introduce displaying the name in the Communications history e.g. email sent to First Name last name emailaddress

Reset the Reviewed designation once a user has selected Save against the screening checklist criteria

Allow the users to unmark the reviewed designation in case it was accidentally saved.
