Remove an additional information form sent to applicant

When an applicant is sent an additional information form, you cannot remove the form if it has been allocated incorrectly. Would be useful to be able to remove it so it is no longer seen on the applicant's profile if it was sent in error.

    also allow additional forms to be removed once a job has been closed

    Encountered this issue today! This would be super helpful as we have to either complete the correct form on the applicant's behalf, or make sure we walk through the process with them so they only fill out the correct form.
