Allow for more employee fields to be shown on the printed performance appraisal

Allow for more employee fields to be shown on the top of the printed performance appraisal. For example, so we can complete a bulk upload process to our enterprise imaging system, OnBase. We need to show the supervisor name, supervisor id, and department of the employee. We index all our performance appraisals by these fields, in addition to the document date, employee name and employee ID.
