Our solution - Put a field at the end of our application where they certify that they are providing truthful accurate information. It provides instructional text that says: "Please enter your full legal name as it appears on your government issued documents such as a driver's license, social security card, or passport." The field is in the agreement section of our application forms and is an open text field.

What is the business process you are trying to achieve?
Merge fields from forms for the offer letters Some background - Individuals were not using their legal names when completing their applications. For example someone is named Alexandra and types Allie for their first name. In return our offers were then addressed to Allie instead of Alexandra. It's not the end of the world but we would prefer to address the offer to the name that is listed on their legal documents they will use for their I9.
