Ask for a reason why people are "removing" (cancelling) themselves from a learning activity/development item

After the system asks "are you sure you wish to remove this development plan" giving them a list of checkboxes with options as to why they are cancelling "eg - unwell, workload, no longer relevant to my development plan etc (this should be customised by the organisation. And then be able to report on it and see how much notice they gave when removing themselves so that people who "remove" or cancel themselves with no notice, we can track people who repeat this behaviour.

What is the business process you are trying to achieve?

Wanting to understand trends as to why people are cancelling from learning sessions and also how much notice they are providing. Would like to be able to report on this.

    This would be really useful to our organisation as well. We currently trying to use the "Score" field as a very clunky way to record how much notice people provide of their cancellation. The reason for cancellation is also really useful. The ability to track repeat offenders is very important to us.
